# Prepare a medication review _with your pharmacist_

Use cabinet details and dose history to arrive with a current product list, clear uncertainties, and a short set of questions.

Moodwell Updated September 20, 2026 3 minute read

In brief

Check the current product list against your containers, distinguish prescribed schedules from recorded use, and write down the questions you cannot resolve. Bring the original labels and review the relevant history during the conversation.

## Start with the whole current list

FDA guidance recommends keeping a list that includes prescription medicines, over-the-counter products, vitamins, and supplements. Before a pharmacist conversation, compare that list with the products you currently use. Occasional items can be easy to omit when you think only about a daily routine.

Moodwell can help you revisit saved cabinet details, but the cabinet only contains what you added and maintained. Look for products that were never entered, old records that no longer describe current use, and names you cannot confidently match to a container. Keep those discrepancies visible for the conversation.

## Separate instructions, records, and questions

On a short paper note or in your own notes, give each item three headings: instructions, recorded use, and question. Copy instructions from the source you follow. Use dose history for dates or events you logged. Put uncertainty in the question area instead of resolving it through guesswork.

This is a preparation method, not an automatic report from Moodwell. You can open the relevant item history on your device during the appointment. Bring the containers or labels when practical, especially if the issue concerns a particular formulation or a supplement ingredient that was not captured clearly.

## Example: replace a broad concern with two precise questions

Suppose your starting concern is that the cabinet feels confusing. On review, you find a supplement with incomplete ingredient coverage and a medication schedule whose wording you do not recognize. These are two specific questions, with different supporting information.

For the supplement, bring the label and ask about its place in the full product list. For the medication, show the saved schedule alongside the current instructions and ask what should be recorded. Do not use the appointment preparation itself to decide on a new schedule or reinterpret a treatment instruction.

## Keep the preparation small enough to finish

1. 1

    ### Check product identities

Compare current names, forms, and strengths with the containers. Mark anything you cannot verify without assistance.

2. 2

    ### Choose the relevant dates

Open the history period connected to your question, rather than trying to summarize every record you have ever saved.

3. 3

    ### Write the main questions

Use one sentence for each uncertainty and identify the item or date involved so the pharmacist can follow it.

4. 4

    ### Bring the source material

Keep labels and your device available. Do not rely on memory to reconstruct a small ingredient list during the discussion.

## Update records from the answer you receive

After the conversation, review which changes concern the cabinet, which concern the schedule, and which simply clarify your understanding. Update only the records that need changing. Past intake should continue to describe what happened at the time.

If you are unsure how an instruction translates into an app field, ask for clarification rather than choosing a close-looking option. A precise unanswered question is more useful than a confident record of something you did not understand.
